Yes! I would like to sponsor the 2008 PwP Golf Outing at the following level:

Sponsors in place by July 15, 2008 will be listed in the program. 

Please check your Sponsorship Level

               Event Sponsor ($500 +) – Company will be represented as an event sponsor.  All banners, scorecards, flyers, etc will contain the company’s name.  The company will also be recognized during the dinner and auction after the event.  If you wish to have a representative from the company at the event, we will provide a booth at no cost.  

               Hole Sponsor ($75 - $499) – Company will be represented as a hole sponsor on an assigned hole.  For that hole, the company may supply any type of advertising that will be placed on the hole’s tee box.  The company will also be recognized by a banner in the banquet hall containing all hole sponsors.  Any type of company flyers, business cards, etc., may be placed on one main table representing all Hole Sponsors.

               Cart Sponsor ($30 - $74) – Company will be represented as a cart sponsor on an assigned golf carts to be used by the golfers.  On that cart, a sign will represent that the cart is sponsored by your company.  The company will also be recognized by a banner in the banquet hall containing all cart sponsors.  Any type of company flyers, business cards, etc., may be placed on one main table representing all Cart Sponsors.

               General Donation (Any) – If you do not wish to be involved with the event but would still like to make a donation.  Please indicate if you wish to be recognized at the event and the amount of your donation.
